1.  Register on-line to participate at: www.healthylombard.org/2014flatapple or in person at the Healthy Lombard table. *

2.  You will receive a copy of our logo when your registration is confirmed. You can print it out from the email, or if you click on the logo when you open the confirmation on your cell phone or computer, it will download it into your photos.

3.  Pick up a “Bite of the Apple Raffle Ticket from the Healthy Lombard Table for credit for each of the activities below:

• Barn Raising
• Square Dancing
• Scavenger Hunt

 4.  Have the coordinator of each activity sign you ticket in the small white spot on the ticket.

 5.  Fill in the other ticket information and return it to the Healthy Lombard table.

 Reminder:  If you take a Healthy Selfie”photo during the taping and send the photos to us at [email protected] and we will fill out two e-tickets for you.
